I've seen exactly one study that clearly showed that prehospital fluid (saline of course, not blood) resuscitation increases mortality and complications. The response has been, including 30 years later, to simply reduce the amounts of fluid, without any evidence that this does not still kill patients instead of helping in any way. And note this has nothing to do with IV fluid in hospital for dehydration reasons. Which is another reason there is no evidence that this is not detrimental in the acute blood loss case.

Just got hired from a medical floor to an ICU position starting in a few months. I'll be watching all these videos. Yours are by far the clearest and best videos to learn from.

I believe you made a mistake. CO2 vasodilates, it doesn't vasoconstrict - @14:48. When you have a patient with increased ICP, you sometimes force them to by HYPOcapnic, so that it causes vasoconstriction and gets the volume of blood out of the skull.

Thank you for the video, but actually "T wave opposite R" is not like a constant feature of PVC. There are different ECG leads and T will have different form in different leads. You can just say that QRS will have different form from usual SA complexes 🙌

I do have a question if you can help me understand, and please correct me if i am not interpreting this correctly: When the problem is failure to pace, no pacer spikes, the problem could be oversensing, so the sensitivity is high (mV low/ fence is low) so we have to increase the mV (aka bring the fence up) which will lower that sensitivity But when the problem is failure to sense, oversensing is when sensitivity is low (mV high/ fence is high) so we have to decrease the mV (bring the fence up) which will increase the sensitivity This is confusing for me because oversensing would have two contradicting definitions. Thank you!

Don’t give BB immediately after diagnosing stemi, studies showing benefit are from before the reperfusion era. Modern studies show essentially no benefit and increased risk. They need to be started within 24hrs but not in the acute management.
